Prior to your first appointment please print and fill out the files requested by Dr. Giurelli The first five files are always required for a new client. If you need to fill out the last two files, Dr. Giurelli will inform you.

Click on the Initial Paperwork menu option to show the drop down menu containing the seven pdf files.
